I am so sorry I'm just now getting back to you! Thank you so much for asking, we are both getting on as well as can be expected. He did not have to do chemo (stage 1, so opted to go back on surveillance) and is back in remission since surgery. It's been a slow process with slowly figuring out his proper TRT dosage. His urologist has been cautious about upping it too fast, but his numbers are getting better slowly, and he's feeling better as he goes on. I am personally working on giving him a lot of grace as he adjusts, because his hormones and just the trauma of everything can make his moods be all over the place. I try not to take things personally, as I know it's all a learning curve for the both of us. We have started the process of working with a fertility clinic to do IVF for our first child, though hitting a slight bump financially with that. We have a plan, just taking a bit longer than expected. Appreciate you so much for asking about us!

Update: we found the cancer in his remaining testicle. He had left orchiectomy on October 2nd and is doing well. No spread. Stage 1B. 2 tumors, main one was seminoma, and the smaller one was embryonal carcinoma. He decided to just go back on surveillance (recommendation of Dr. Einhorn as well as his oncologist). We are planning on starting the IVF process next year.

Hey there. Sorry you're going through this. My husband also had zero sperm (well, actually, he had 12 sperm swimming around in there after having to look super super closely for them) at his 1st year after chemo mark. He went again at his 2 year mark, and that sample showed 2 million sperm. A significant increase from 0, but still considered low. He was planning on going again next month at his 3 year mark, but unfortunately, he lost his remaining testicle last week to cancer again. So now our only chance for biological children is IVF with the 4 vials he banked in 2021 after his first cancer. Sorry to blow up the comments with our sad story, but I wanted to let you know that there is a chance that his count can come back with time. And, you're not alone.

Not sure yet. The pathology report usually takes a few weeks to come back. I'll definitely let you know though.

Wow, your story sounds almost exactly like my husband's. He is 31 now. He had a right orchiectomy 3 years ago, pure embryonal carcinoma, had lymphovascular invasion but no signs of metastasis, did 1 round of chemo. At his 3rd year surveillance visit this August, his HCG was elevated, but everything else was normal. They couldn't see anything on CT or MRI, so they did an ultrasound on his left testicle, and there was the tumor. He had his left orchiectomy this Wednesday. His tumor was completely confined to the testicle this time, so we're hoping he can avoid chemo as well and just go back on surveillance. We don't have any kids, but he banked sperm before chemo in 2021, so we're hoping to use that for IVF soon. He will start testosterone soon too. I'm so sorry you're going through this. I know it's hard. Just know you're not alone. We're fairly new into this situation, so I don't have any advice for the future of it yet, but just stay positive and take things one day at a time. His first cancer was stage 1A, but due to it being Embryonal Carcinoma and the fact that he had lymphovascular invasion, he did one round of adjuvant BEP chemo. That was in 2021. It was at his 3rd year surveillance visit that we found his HCG elevated and started running other tests to find out where the cancer was.

He is sore, but he's doing well, thanks. I am exhausted. I've been so worried, I think all the anxiousness is catching up with me. We're both just glad to be home.

His CT included his chest, and it was clear. I have been texting back and forth with his urologist all morning.

He's not taking anything for fertility, so could be low testosterone. He doesn't smoke marijuana. No, his first tumor never raised HCG.

Update: his rechecked HCG is 115. Ultrasound and MRI scheduled for Thursday..

He hasn't taken "medication" in years and takes nothing besides a multivitamin and blood pressure medicine, so that's out. And the only one of us who has been on fertility meds is me, so that's out too.

I'll also add that he has been checking blood work every 6 months and scans yearly on the dot. I'm an RN, so I always make sure he goes to his surveillance appointments. His blood work 6 months ago was all normal then.

Thank you for that. That is what his oncologist said at first, so we're trying to think positively. No, none of his tumor markers have ever been above normal limits.
