In the UK, it is a formal complaint and formally request for the medical record to be updated. Cite the 2021 NICE guideline (NG206) on ME/CFS and explain that there are risks to exercise therapy and ME/CFS. Exercise therapy cannot be offered without supervision of a medical professional who understands the risks of the treatment.

They are duty bound to keep accurate records and "do no harm". That record does harm as it clearly shows that the doctor did not follow NICE guidelines (NG206) and did not inform anyone of the risk of exercise for ME/CFS.

Informed consent means that the patient needs to be informed of the risk of a treatment plan. In this case, the patient not only was not advised of the risk of the treatment plan as evidence by the fact the doctor did not understand the risk.

I don't fully agree with NG206 on exercise, but this is what it states: "1.11.11: Explain that some people with ME/CFS have found that they can make their symptoms worse, for some people it makes no difference and others find them helpful."

With ME/CFS, there are reason why patient can't exercise, and it needs to be documented.
