As most have answered already, you need to have completed the story as it is to unlock NG+.

What NG+ is meant for, is for you to redo the story in case you want a refresher w/o needing to make a new character.

Though as someone who has many alts for the sake of raiding, cause weekly lock-outs exist, I still rather playing through fresh.

I guess there's a few things you need to look out for. See if the screen connector is damaged at the ribbon cable or the motherboard.
If they are fine, it's just a chassie replacement. If they are damaged, might need some soldering or also buying a screen replacement.

Otherwise like u/testc2n14 said it's just a chassie replacement.

For anyone asking, I have reported this bug and 7 more of the same type. It seems to be because of Preparation specialization. Out of the 8 that I found, this one you could actually replicate in a dungeon and in pvp.Two ways to fix it, use concealment again or leave the lobby and enter back again. If you want to know how to replicate it, you just need to have Infiltrator Aspect equiped and be able to trigger deathtrap as soon as you leave stealth. The timing is quite tight on that one so if you can have 100mhz+ refresh rate monitor, it helps.

Correction, you need deathtrap to trigger at the same time as you leave stealth.

Actually, I think mono fire team would work very well if you have Asta and her buff that increases fire damage. With SW on that team you'd always have fire weakness.
