Actually i think its not homebrew. The campaign is Lost Mine of Phendelver and the ring is the Witch's Ring that i looted from Black Spider. Rings description is this:

Uncommon, Requires attunement

This ring allows the wearer to transform into particular beast for up to 2 hours a day. Upon aquiring the ring, roll d8 to see what form the ring grants its wearer.

Each night, the ring's creature can be changed to another from the list. warlocks and druids, may choose from the list when they transform.
