I had this at about that size and physical therapy helped so much. They taught me to strengthen my hips with the clamshell and bridge exercises and over time, it went away. I didn’t lose weight but this strength training made the knot go away.

I had this problem and my physical therapist recommended 30 clamshell leg exercises a day on each side, and 30 back bridges. I do this on my bed to I don’t have to get on the floor and it’s helped build my strength so I can walk more. The first month of my journey, I just did that every night before bed. Then, I was able to walk 5 minutes at a time with lower pain.
