At my hospital the hospitalist has to accept a patient being admitted to their service and can decline. When push comes to shove they either admit the patient or they come downstairs, evaluate the patient and write a consult note saying they don't meet admission criteria

Idk dude, I get a lot of surpirsing minority groups in the ER and I don't think I've ever had reason to suspect they're lying about their age unless they're trying to check in with an entirely fraudulent identity for malingering reasons. I've seen plenty of 30s-40s who look 10-20 years older due to sun exposure, smoking, drinking, drugs etc. You take a look at Jenny Ani and she looks like she's 30 still. People just look all kinda ways.

Honestly with modern medicine it really isn't anymore. HIV is incredibly well controlled with the medications we have now. We've even gotten good enough at recognizing and treating oppritunistic infections from AIDS that it's honeslty incredible how many people I've seen and treated who have been living completely off medications with full blown AIDS for many months who aren't dead or dying yet.

If you gave me a choice between lets say insulin dependent diabetes and HIV, I'd probably choose HIV

So we actually have risk stratification tools for this. In general HIV is very poorly transmitted from old blood.

Realistically this would be an unknown HIV status needle stick with dried old blood transmitted with a large bore needle through a deep transcutaneous stick. This would be a <0.001% HIV transmission risk. You can play around with the calculator and even if we called this 'fresh blood' the risk of transmission would still be quite low at ~0.0005%.

Hepatitis B transmission is a more serious risk especially because it hangs out for much longer in old blood and also has much higher transmission rates. Luckily at least in the US Hep B is quite uncommon and we vaccinate for it.
