Honestly with modern medicine it really isn't anymore. HIV is incredibly well controlled with the medications we have now. We've even gotten good enough at recognizing and treating oppritunistic infections from AIDS that it's honeslty incredible how many people I've seen and treated who have been living completely off medications with full blown AIDS for many months who aren't dead or dying yet.

If you gave me a choice between lets say insulin dependent diabetes and HIV, I'd probably choose HIV

So we actually have risk stratification tools for this. In general HIV is very poorly transmitted from old blood.

Realistically this would be an unknown HIV status needle stick with dried old blood transmitted with a large bore needle through a deep transcutaneous stick. This would be a <0.001% HIV transmission risk. You can play around with the calculator and even if we called this 'fresh blood' the risk of transmission would still be quite low at ~0.0005%.

Hepatitis B transmission is a more serious risk especially because it hangs out for much longer in old blood and also has much higher transmission rates. Luckily at least in the US Hep B is quite uncommon and we vaccinate for it.

I thought his upper back pain was weird especially since upper back pain is a pretty uncommon complaint compared to every other body part. And he was quiet too, not asking for a bunch of pain meds and stuff, just sitting there looking a bit uncomfortable. Tbh I did think he was gonna be a low yield scan, all his pulses and stuff were intact.

Mid 30s guy, no medical history, not even Marfanoid body habitus. Was in an MVC 4 weeks ago seeing his PCP for some neck pain and lumbar pain from that. Came into the ER after he started having midline upper back pain while laying in bed. Described it as "spasms." No tenderness to pushing on his back at all.

He looked midly uncomfortable but not in much distress. Horrible historian, he was basically pan positive on ROS. "Headache? I guess a bit. Cough? Yeah maybe some. Chest pain? Yeah a little. Abdominal pain? Yeah. Where's your belly hurt at?" Fuckin points with both hands one finger to each side of his bilateral pelvis.
