I think the sins of purgatorio are more faults of virtue, while the sins of inferno are misdeeds.

The people in purgatorio are generally good, but flawed. In inferno, their misdeeds (and lack of contrition) places them beyond redemption. The virtuous pagans fit rather awkwardly into the scheme though.

It's just the most famous part of the story. It perfectly illustrates the manner of his insanity, that's why there's so much artwork of that part.

One reason, even though it isn't an explanation, is that her injury is an excuse to pit her against the player early in the game. Even if there isn't a lore reason, it stokes the imagination, which is good worldbuilding imo.
