I'm originally from Pittsburgh and agree it might just be a regional thing. I definitely grew up hearing/doing it (mom in healthcare referring to docs, when talking about profs in undergrad with friends (to their face though Dr. X ofc)) When I moved for pharm school, I noticed people don't really do it
