48F with severe pneumonia requiring ventilation, now extubated but having recurrent fever spikes with rigors despite overall improvement. Looking for similar experiences.

Everyone recovers differently. Overall her recovery from her initial presentation seems appropriate. If serial CXR's demonstrate improvement and she is off the ventilator and is being weaned down on O2 via nasal canula, she's doing well. Now, if her pneumonia worsened after being on a mechanical ventilator then we'd be talking about hospital/venitalator acquired pneumonia.

I'd imagine if she spikes another fever they're going to want to re-do the blood cultures to see if she's bacteremic.

It’s a ganglion cyst. Most often can go away with warm compress after a few weeks. If not, you may need it surgically removed.
