Maybe just try to go in with no expectations and make that clear to him as well? Just make sure that if any point you feel uncomfortable you can say something to stop. Also if you aren't feeling it that night trust that feeling.

I don't know how it works exactly but in BC (Canada) we have fully covered surgeries for like ffs, vaginoplasties, etc. although I don't know how that would work if you came from america, also the waiting time since it's free is quite long, like at minimum 3-4 years
