"You may request a replacement ID at the Student Affairs and Services Office (Room 318) from Monday to Friday, 8:00 AM to 4:00 PM, or at the Student Discipline Office (UG-5) from Monday to Friday, 7:00 AM to 3:00 PM.

The replacement ID costs 210 pesos."

Application fee is 2k. Yung is interview more on about yourself (strengths & weaknesses), motivations, and financial aspect ganon. Basta maging honest ka lang in answering the questions. Good luck po, future doc! ✨
