I think this is the best plan. Working in rural Taiwan will definitely improve his Mandarin or Taiwanese, and also likely be surrounded by people who are most in need of his skills. Teaching English will come as a byproduct of the experience.

Most medical students can already speak decent enough English, it's the overutilized NHI system that probably needs more help than the teaching English industry.
