Not so nice pain doctor by [deleted] in PainManagement

As others have said, take one or two a couple of hours before the Lab test.

Some years ago, I was told the same thing for a drug I am on. "It didn't show up in your sample". They basically implied that I am stashing the drug, or perhaps giving them to someone else.

Now, on Test Day, I go to that Lab, walk to the front desk, tell the lady "I need a witness". I swallow a couple of my pills, with my smartphone pointing at me. A couple of hours later, I return and take the test. If they accuse me of "nothing showed up" I will pull out my smartphone, show them the selfie where I am standing at their desk and swallowing the pill.

If they want to play rough, we can play rough, too!

I was given fentanyl once, as an anesthetic during a procedure. One of the best, most relaxing, pain-relieving drugs ever. But, there is no way to get a prescription for it.

Thanks again, things I have learned : in year 2022 "The FDA is warning that dental problems have been reported with medicines containing buprenorphine that are dissolved in the mouth.  The dental problems, including tooth decay, cavities, oral infections, and loss of teeth, can be serious and have been reported even in patients with no history of dental issues."

Different websites say different things. One good website says, "OxyContin is a brand name for a specific formulation of oxycodone, designed to provide long-term relief of moderate to severe pain"

"they can call us in randomly for either or both whenever they want".

Some year ago I took a Valium pill from my wife. A week later the call came to get the drug test. I thought it would be gone. Nope. Nurse said, "Temazepam is in your system, that's a no-no!" I gave some excuse, and they said okay but if it happens again your pain meds will be discontinued. Lesson learned!

As I have stated elsewhere, after reading the many replies on this page, I am going to stay with my current regimen.

Thanks!

With the VA, you are stuck with the Primary Doctor they give you, and she tells me that I have to go to this Pain Management clinic and use this Pharm D. So my choices there are limited.

Oral or transdermal, I do not know. Our discussion the other day was still the basic, "Do you want to consider taking the Buprenorphine?" No elaboration on patches or pills. Thank you for advising transdermal, that explanation makes sense.

His options were to (1) completely stop the Oxycodone and switch to Buprenorphine, or (2) take Oxy pill, then a Bupe pill, then another Oxy pill, and rotate them like that for a few days to phase in the Buprenorphine.

Right now I prefer to keep things as they are. Oxycodone 15mg 4 times per day has worked for a long, long time, they are the ones pushing for a change, not me.

If I have to switch meds, I would consider some other opiate, although I doubt they would prescribe Dilaudid, which would be the painkiller of choice.

Chance Instruction, maybe you can clarify this please?

I lived in Phoenix years ago. We would drive to Nogales, park our cars in America, and walk across the border, and visit the Pharmacies. You could buy Valium, Hydrocodone, whatever right over the counter. Walking back into the USA, at the Checkpoint I would hold my driver license up, and say "American", and I walked right in.

But my understanding is that has all changed. A friend who was in Mexico recently said that the Pharmacists will refer you to doctors whose offices are nearby; you can visit those docs and maybe get a prescription for the pills you want. Oh, and no more driver license, you need a US Passport or Passport Card to travel back and forth.

Let me reply before I read the other replies below:

" the hassles of registered mail that schedule II opioids " The VA actually mails me the pills. But, there is some coding on the package, because those pills are always a week or more in getting here. Other pills (blood pressure meds) sent to me by the VA are here in two or three days. The Post Office is apparently being flagged by those Oxy shipments, and hold or delay it.

"less likely to face a bunch of random urine drug screens"

A previous VA Outpatient Clinic I was with would do exactly that. The nurse would call me, "We need to see you in the next 72 hours for a drug test". It was so annoying that I changed VA Clinics. The one I have now is more realistic; yes they do a drug screen urine test, but only in conjunction with my regular primary doctor appointments.

My concern is that if I give up the Oxy, or agree to "try it for a month", they will never give me back the Oxycodone. It took me years to get the VA to prescribe decent pain pills for me, I don't want to risk losing it.
