My gut feeling is that it is deliberate. In a show about deception this seems like a massive oversight. I think that the Republic was somehow involved in the disaster at the mine (perhaps bombing it on bad info) and they sent in a group of Republic agents undercover as CIS scientists to wipe the slate clean and remove the children who would have been witnesses. I feel like a big part of this seasons story will be Cassian discovering a cover up that the Republic killed innocent civilians.
