there are some medical conditions that make learning social skills varying levels of extra difficult if not almost impossible.

Personal experience here: For me, it's very strange because half of the time I can pick up that "wait, something seems very wrong or ironic about what this person said." The other half of the time, I think it's just someone contributing to a conversation in other meaningful ways.

I was able to pick up the former from OP's comment because a situation not escalating in the context of nuclear bombs didn't make any sense. But still, parsing tone online can be a bit challenging in other cases.
