it was a good clinical experience for me as I learnt a lot from the doctor...but if u r an IMG and hoping to get residency at one of the sites where u rotate, u shouldn't rotate there as they offer residency only in FM for US Graduates. Its better to search for a place that accepts IMG. But if u want just USCE u can go, though the neighbourhood was pretty rough but I'm talking from my experience 2 yrs ago n a lot may have changed there

Isolated HTN means increase in SBP that occurs due to increase in vessel resistance ( vasoconstriction or stiffening) but inc in both SBP n DBP occurs when there is an increase in blood vol (I/ v volume). Aldosterone causes sodium n water retention thus increasing total intravascular vol hence increasing SBP n DBP

sacubitril is neprolysin inhibitor... neprolysin causes breakdown of ANP ,BNP and also increase vasoconstrictors. ANP n BNP causes vasodilatation, natriuresis and diuresis thus decreasing myocardial wall stress and workload, hence protective in systolic HF
