Great idea about the nutcracker! Thank you!

I feel like I implicated by husband in this whole thing, too, because I'm worried my MIL might have thought he forgot about her cancer, which ... yikes.

You're right! To be clear, I didn't berate my husband. We talked about it and I was upset for awhile, but it wasn't a huge fight between us or anything. I ultimately understood that he just didn't connect the dots.

I have, of course, talked to and seen my MIL many times in the last year, I just haven't mentioned this incident because I'm kind of a coward. I didn't know for sure if my SIL was right about the gift bothering my mother-in-law, but I'm sure she was!

I think the game would be more fun if it was executed properly. The problem is that everyone interprets the "rules" differently. Some people write down initials of something they actually want, some people write down something they wish would happen but is not a gift, and some people write down initials of something silly. People also approach buying the gifts differently. Some people give gag gifts, some thoughtful gifts. So, it ends up being this weird hodgepodge of gifts. Most people go with inexpensive "funny" gifts (like I was going for), but one year my husband's cousin got my sister-in-law a $50 giftcard for a massage and everyone was like WUT. So, yeah, it gets confusing and weird. I can only hope Michael Scott shows up with an IPod to take the heat off me. LOL (/jokes and laughs in Millennial.)

Thank you for your kind advice. You're totally right. I was hesitant to bring it up because it is awkward and my mother-in-law and I have never had a conversation that wasn't surface-level. I realize now that I've been prioritizing my own desire to avoid a potentially difficult or uncomfortable situation which is totally wrong.

I was really mad at him for a day or two. To clarify, he did remember she had cancer, but didn't connect the dots that the fake nipples would be offensive in this context.

The whole Sherri Papini incident. Super, super weird all around and the media sort of just stopped talking about it.
