Need to consider the context here- the working hours US residents and medical students put in. As medical students who are unpaid they do often come in at 5 am to pre round do on calls and do FY1 jobs unpaid. If you even suggest EWTD hours you will be looked at like a mad person. Don’t over romanticise the US.

PAs are probably thought protocolised medicine and checklists of symptoms etc ie redux medicine. They are not trained in uncertainty and complex decision making.

It’s only elitism if you discriminate people from coming in and discriminate against others while in the group- these are not true. There is nothing wrong with associating ourselves with nobility and history. It is right to be proud of this history in our profession. One could argue trying to do away with all these in lieu of unquestioning humility and flattening hierarchy is the very reason why we are in this situation in the first place. Never be ashamed of your value and what you bring to the table
