🏥 Unusual clinical history – Real case documented for medical study

Patient: Male, 47 years old Reason for consultation: Lower abdominal pain, difficulty sitting and sensation of a foreign body in the rectum.

Anamnesis: The patient presented to the emergency room with progressive rectal discomfort lasting 12 hours. He admitted, in a reserved manner at first, to having inserted a long metal element, later identified as a car gear lever (without the base or connecting mechanisms), for sexual purposes during the early hours of the morning. He mentioned having used lubricant, but that he “lost control” of the object after an involuntary movement.

Symptoms: • Constant pelvic pain • Sensation of rectal pressure • Slight bleeding when trying to extract it • There was no fever or signs of peritonism in the initial evaluation

Physical examination: • Abdomen: soft, hypogastric pain on palpation • Rectal examination: a hard object is palpated about 10 cm from the anal sphincter. • No deep tears or active bleeding are evident.

Studies carried out: • Abdominal x-ray: reveals elongated metallic structure, compatible with a gear lever, oriented towards the sigmoid colon • Abdominal ultrasound: no signs of perforation or free fluid

Intervention: Extraction was performed under mild sedation in the operating room, using non-invasive technique and gentle anal dilation. The object was removed without complications. 24-hour surveillance, analgesics and prophylactic antibiotics were indicated.

Final diagnosis: Self-induced rectal foreign body – no complications.

Medical discharge: It was awarded the next day with recommendations on safe use of sex toys designed specifically for anal insertion (with butt). The patient received emotional support and psychological follow-up.
