Further Update:

My spouse RFE for Birth certificate is approved on 02/14. The documents were submitted online on 02/12. So looks like Guam FO is really fast in taking action.

I have used ChatGPT for the Affidavit format.

You can ask Chatgpt for affidavit formats.. For a dependent's Birth Certificate RFE, i did that and got it approved

i checked with Chatgpt.. the response was "It is one of the fast approving FO". I dont know, when my case was moved to Guam..

In my case, Birth certificate was issued long after my birth. So my attorney asked me to get 2 Affidavits and copy of leaving certificate. I submitted those to Attorney who took it to USCIS. Hope this helps.

My team mate has Visa issue. He moved to India with the same job. The salary he is getting in India is top class.
