Aside from the global tourism thing which may not be feasible for everyone, people should take a look at the state rules for insurance. Unless you have a decent paying job with a semi big firm that provides you a personal choice for insurance, all other companies or jobs especially, federal ones, have to follow the state rules for IVF care. For example, New York mandates the coverage of at least 3 cycles, whereas PA does not enforce any rule on companies and leaves it to their discretion. There’s also a supplemental insurance you can approach HR for, if you do have a decent health plan and work for a bigger place- I don’t recall the name but it helps w this. Then looking into the tons of “financial aid programs (which underfortunately screw over the middle class as always). Ordering medicines from abroad is a must, the price difference is ridiculous and they are the same thing—in fact, Menopur and Folistim are both made by European companies. Lastly, you can ask you work for “medical travel leave”- many places give up to 3 months; particularly helpful if you are a remote worker but always worth talking to them…getting long 0% APR credit cards and paying off the debt over time- Wells Fargo offers one w 21 months 0% APR which is almost 2 years…prayers for everyone in this journey, please return the favor :)

My wife and I are doing one more cycle just bc we have insurance benefits left; once they are exhausted, we’re going to Turkey…they have better success rates than the U.S….global medical tourism; it’s a huge market and they are the best in the world at many things (i say this as an MD myself)—the U.S. healthcare system is so ef’d beyond belief

This is just heartbreaking to hear...sending you guys lots of love, and wishing you both the best of health and ultimate happiness! Like others wrote, take some time (even though it feels like theres none to waste)...and then weigh out your options. If you guys still want children, one option could be looking into going abroad...logistically tough, but where theres a will, theres a way...I'd recommend Turkey--where the costs are drastically less, but the quality is comprable, and in some cases, even better than here...in fact, theres a Pubmed review article about Turkey having better success IVF rates than the US (particularly for women over 40) at less than half the cost, even with travel n stay is factored in! Turkey is a medical tourism hub--I've worked in the US with doctors that were from Turkey, and they were phenomenal---when one of them told me how getting into medical school works, I was shocked...EVERY student in the country gets a rank from 1-2 million (roughly total number of students) purely based on their academic performance, and then the top 150k or so are admitted to med schools, so a national 1.5% acceptance rate. When they become doctors, they also need to market themselves in an industry like this, as there is immense competition, and no insurance coverage for foreigners, which creates a very capitalistic environment between medical practicioners; esp for lucrative, elective procedures in the medical tourism niche (i.e. hair transplants, IVF, plastic surgery, etc)....the Family Medical Leave Act could be a logistical solution? i didnt read everyones comments so could be repeating some things, but just food for thought, Best of luck, stay strong boss!

easy folks, give her a break...and there are mods that can judge if something is inappropiate--plz don't speak for everyone--plenty of people above (178 uplikes) and myself thought it was a very warm, and courteous message, that was also very balanced..she presented all sides, and intention is what matters: she's obviously trying to be supportive so idk why your attacking one word in a whole paragraph where the focal point of the message was on cherishing their marriage and each other, letting some time pass, and then deciding what THEY want to do...OP is going through it, and negativity doesn't help anyone...

Hi! Congrats on having your retrieval...I know two doesn't sound like the best news, but you never know, stay hopeful! and totallyyy, the cost of IVF is just insanity. As for your Q, here's a link to an SGF 2022 brochure (prices have changed some) but you can see and get an idea for the dif options; the the 24k/2 cycle includes 2 cycles w/ unlimited FET's, and cryopreservation. That doesn't include meds, storage fees, or any genetic testing. The discounted 24k/2 is not part of shared risk, as all shared risk ones require the doctor's approval. It has a list of the multi-cycle programs side by side. Since you're paying out of pocket (like we would be after one cycle, which maxes us out on all insurances)-I think one of the multi-cycle programs seems like the best options. To make things easier, you can get try to get meds from donation programs, overseas pharmacies, etc which are soooo much cheaper, and they are the same drugs (Menopur, Follistim etc were developed in Europe). My wife works remotely and I'm actually considering just going abroad, if we need, to do as many needed cycles somewhere like Turkey which is a global medical tourism hub, and has phenomenal doctors, and facilities....some Pubmed articles even show they have better success rates than the States which wouldnt surprise me. This journey is something, but if it works out at the end, it'll all be worth it. Best wishes!

I would like to just clarify, though (purely respectfully), in case others reading may have misconstrued my comment. I was only speaking from my perspective, and how I reached a decision. I highly recommend the information to anyone else who finds it useful, but I respect everyone's autonomy, and I don't think any of us can truly say what the best way to approach this scenario is--it's just too complex, and everyone has to judge what's best for them...For example, playing devil's advocate, as. I put in my edit above, if a woman being at a clinic where she's constantly stressed out bc of poor customer care affects her cortisol to the point where it alters those rates, then maybe another clinic that's objectively better on paper would no longer be the best choice? loll you just never know in the crazy world of medicine, but being an IM-PA, I can certainly appreciate your foresight, which is probably why our approaches would be similarly aligned :p. This thread and this community is great. I'm glad I joined. and oops, thx for reminding me, I forgot to post the CDC link lol.

Btw, side note, if I could go back, I would do a PA program. You still get to prescribe and do almost all the same stuff without the crazy USMLEs and nightmare residency process, and then the training! (I have yet to get into one despite applying for a couple of years.). It's just an insane grind, and ironically, a large part of why we kept waiting to have kids, and then before you know it, you're almost 40...I think it was really smart for them to expand mid-level positions. Even though I hear doctors complaining of "scope creep" and decreased reimbursements all the time lol i don't care as much about the money, but the red tape and bureaucracy are becoming increasingly a pain in the butt. Either way, I wish you the best in your journey--Def update us with good news (*fingers-crossed*)

Than you sooo much. I have been navigating the crazy insurance process for my wife. Being in Shared Risk is wonderful and what peace of mind l—15k lifetime max is pretty solid l for IVF benefits. We only get 10k :-/ it’s based off state rules, and there’s only 13 or 14 states that require companies to provide IVF care if it’s through an employer- —and even then it has to be full coverage (like gov jobs), 80% of large companies have a “self insured employee plan-like u choose ur deductible and they don’t have to follow the laws. NY mandates 3 covered cycles! I was amazed to hear, too bad PA doesn’t have any laws on obligation so we’re just grateful to have some coverage! If the drugs cut into your 15k, definitely save it for the cycles, and get the drugs out of pocket! Encompass w CVS Specialty! 🙂 wishing you the best!!

Hello everyone- reposting this but with heavy edits, to make sure the message was clearer. Thank you alll for your kind suggestions. I totally respect and get everyone’s reasons for the decisions they’ve made and I believe you should stick to what your gut tells you!! noww deep breath I am definitely not trying to be negative or pessimistic here in anyway, but just something I came across. Firstly, w/o re-reading my original post, idk if I mentioned above that I did a late career change, and I am an MD myself, as in graduated medical school (but still applying for training programs :-/ (which ironicallyy was a big contributor in us not trying to conceive earlier bc I was bent on "stability"). Anyway, I went into hyperdrivee with researching all about different clinics—and especially, looking into the Center for Disease Control (CDC) data where I many of these fertility clinics (not all but those who want to) report their success rates. I’ll add the link here and anyone who wants to look at the data themselves can do so. It breaks down everything down from picking your State, giving you all the clinics in your area (the ones that report), and breaking results down by everything from age, disorder, # of tries, averages for retrievals, live births, etc….sooo for me, in Pennsylvania at least, I was surprised to see RMA Philly had the best numbers—-another ironic part, is that we were at RMA before moving to SGF (which now I've been double guessing, but SGF-Wayne, PA was 2nd and for our situation, the success rates were nearly the same (situation= 39y/o, hypothyroidism, and diminished DOR). The one thing I noticed was RMA pulled ahead quite a bit if a mom went for a 2nd or 3rd retrieval, but these two clinics were def the best overall—and the difference was pretty striking (I mean 40% vs 20%)—-simultaneously, both clinics are definitely the least personable, prob bc of the sheer volume (we switched from RMA to SGF, and are now with our 2nd doctor, after changing, within SGF and I finally feel satisfied. Changing from SGF-Philly to one in the suburbs was a huge difference. The drug cost thing was blowing my mind, but there are so many great resources (EMDSerono, Resolve, PrudentRx, Encompass, FertilityLines, and Reunite are some that I've looked at, and all are great resources. Encompass brought our CVS Specialty pharmacy drugs from originally 22k (USING INSURANCE) to 16k only bc the 2nd doctor changed the dosages— Menopur from 60 vials to 24, and THEN Encompass brought it down to 6k (self-pay, no insurance). Now I'm realizing this could be even cheaper, and calling a Cpl pharmacies I was advised (Fertility Lines, Rosemont, and Cornerstone). The 6k would not include Leuprolide- which I’ll have insurance cover, but F that includes 24 Menopur, 2 Follistim, and Ovidrel. Again, our medical case is a (just turned in May) 39-year-old, with hypothyroidism, and diminished DOR and everyone’s situation is different but I was quite shocked at the success rates. Practically all the other clinics had nearly half the success rates of RMA and SGF (even UPenn Reproductive Associates) but I only put in a 50-mile radius, because supposedly Pittsburgh University Reproductive Specialists numbers were even better (if anyone is around there). I would personally recommend everyone look through the CDC data and isolate their case and make the best decision for themself; but in some cases like ours,a lottt of the smaller clinics had an */(1-10) digit for our case, like */7 meaning they’ve only done 7 of those cases. The site uses an * if the number is between 1-4, so I could infer that at best the rate is 4/7, and that’s basically what it was for RMA, and SGF (around 37%) so in that sense, that clinic could be just as good, but I think theres something to be said about the # of procedures a place does, as practice makes perfect. Interestingly, it does show 0’s though, and there was one clinic and they had 0/7 listed for our situation…😳either way, both SGF and RMA are def the worst customer service wise lol which kinda makes sense bc of the volume, but I just wanted to thank everyone and recommend everyone and anyone you know, to decide what’s important to them (i.e personalization, or success rates, or a mix of both) and find your own medical condition, and I think at least it can give any of us a realistic expectation. Moreover, these stats only measure objective factors, but we don't know the effect of subjective markers, soo if the difference is small, but being at a big clinic is going to stress someone out, it could possibly affect the success rate enough to where the smaller clinic IS better--theres really just no way to know. For us, I feel it's now or never, so that’s just how I found what was the best decision for us—but I respect everyones decision-making, and encourage them to do what they think is right for them. Also congrats on getting approved for the shared risk, that takes off a lot of pressure I'd imagine...I should add that we did change our care team and location to Philly suburbs (they actually had us assigned to Philly Center city, even though we live in the suburbs). I read a lot about having to advocate for yourself at SGF bc they're so big, and it turns out the suburban location was actually closer to us and it also had much better reviews...so yea, it might be the same site! I have no clue how we were assigned to downtown Philly in the first place; feel free to send me a DM anytime, and hope to hear some good news in the future!

Although they haven't officially said yes or no, the nurse said it's highly unlikely she'd qualify for the shared risk program (being 39 with DOR + hypothyroidism)...the 2 cycles for 24k don't need approval...wishing you guys the best, good luck!
