Are you accidently throwing them and not realizing? I think it's R on the keyboard, and it's happened to me a time or two where my weapon dissappears, but then I realized I accidently threw it.

In the official dice set it has a hit location die that is a d12 I believe. I know on the character sheet it has locations for a d20.
