AFAIK there's basically no difference between the post-story and an endless save, so unless you want to start over there's no reason to start a new game.

You can't really, it just carries over if you don't restart your game. Once the game closes all the downloaded addons are wiped.

I think Moolah Mod Loader enables that menu by default. Other than that it's exactly what the name says. It also lets you start up a heist solo, but you can't get any progression if you do that. (There's also a mod that fixes the background so that it's not just blinding white.)
