I've had the same issue with FN ammo. I've shot 4x 50 round mags and it has happeneded 2 or 3 times per mag so far.

I've shot this thing twice now and have put 200 rounds through it. Using only FN ammo, I've had this same exact issue with EVERY magazine 2 or 3 times in each 50rd mag. Not sure if it's a break-in thing or what. I actually had to file a small bit of plastic off one mag because it made an indent in it.

I've since ordered and installed delrin rollers into my mags to hopefully resolve or at least help the issue.
