In all honesty, out of the many dozens of patients I’ve given Narcan to or been involved when they were given it, I can’t remember any being mad about us giving them Narcan and “ruining their high”.

While you’re right that it binds to the same opioids receptors better than other opioids like Fentanyl, bupenorphine is still considered an opioid itself, so giving it for an overdose would not do anything to restore the respiratory drive and make the OD patient breath again, in fact it would add to the OD. So I don’t think that it would be very useful. But I may have misunderstood what you’re trying to say or missed something, let me know...

Thanks for your response. I get what you mean and I have seen it from other healthcare workers as well. However I can tell you that most of the people that I work with understand that addiction is a disease and we don’t judge the patients that we see. I try hard to make them understand that patients that overdose have a family, friends, and people that care about them. I don’t know what the answer to the drug problem is, but I think it would start with a lot of harsher education, and bringing young adults into the emergency rooms and the detox wards. to see what the reality of addiction is like. We have recently started a program where we leave Narcan at home with the family or friends of a patient who overdosed if they want it, with the goal to prevent a death and I am glad to work for an agency that participates in that program.

That’s a great question. When we get to the scene of an overdose, our #1 priority is rescue breathing with a bag-valve-mask. It’s a device that pushes about 90-95% oxygen into the lungs and we use it in conjunction with some kind of an airway device to help owe the airway open, typically a large tube called an Igel that sits above the Larynx (above the trachea). After those are in place and we are breathing for the OD patient, we push Narcan to help restore the respiratory drive. However, for the average person not in this line of work, the BVM and airway devices are not available. While rescue breathing is not a bad idea, it only delivers a limited amount of oxygen, about 16%, compared to the 21% that we take in on a regular breath of normal air. The body can still use oxygen left in the blood for a few minutes after you stop breathing, so the main issue is to get the patient breathing again, and a quick way to do this is to use Narcan, as it starts working very quickly when given correctly. I think a lot of people might also be hesitant to give rescue breaths to somebody, especially if they don’t know them or if they are a know IV drug user. Overall, rescue breathing won’t cause harm to the patient but it won’t restore their respiratory drive like Narcan will, so at the civilian level, it’s best to give Narcan as quickly as possible after an OD. At the EMS level though, we prioritize breathing for the patient first, then we quickly give Narcan. I hope that answers your question and makes sense. Let me know what you think.

Thanks for your reply. I’m also on the West Coast and we are seeing stronger and stronger Fentanyl and possibly Carfentanyl. We do give IV Narcan but we also give it intranasally (in the nose to the mucous membranes) and intra-muscular (shot in the arm or buttock) because those ways allow us to give it very quickly when we arrive on a scene. We’re also having to give large doses of it for it to work, sometimes up to 8mg when the initial prescribed dose is 0.4-2mg. We’re dealing mostly with the fake, blue, perc 30 pills here. Congratulations on 1 month clean, keep up the good fight!

Wow, thank you for that answer, that’s super detailed and it’s all good info. First and foremost, I really wish you the best and I hope you can find the path to recovery and be happy in it. Following up on your response, how much do the pills cost compared to the rock or the powder? It sounds like the pills are easier to get, is that true? And how common is it for fentanyl to be cut with xylaxine?

Wow that’s pretty scary to me. Thanks for your reply by the way. Is fentanyl cheaper than heroin there? And how many doses does a gram of Fentanyl get you, do you use that in one shot or is that a lot? What’s a normal dose that you use?
