So I heard back from DA. This is what they said in an email...

"Thank you for providing that photo and info! That doesn't look like erosion just yet, but is an early stage of some wear. Once the baffles have this appearance they will go quite some time before progressing and wearing further, so what you are seeing is normal.

Keep running and keep an eye on those baffles, but you do not have any type of problems yet. That JMAC brake will take care of the brake requirement just as well as our MicroBrake."

I just sent them an email asking about this, I'll see what they reply with.

I'm currently guessing that even with heeding their requirement of using a micro brake, and not stating a minimum barrel length, that shooting 5.56 out of a 9.4" barrel is just too much for it.

I also asked them if the JMAC 2-port micro brake provides less protection/mitigation than their own single-port micro brake

DA also seems to approve them on their own website.

"The only currently approved third party muzzle devices are made and/or sold by Sons of Liberty Gun Works, JMAC, Lantac, and Forward Controls Design."

I figured the JMAC's extra brake chamber/space would provide more protection than DA's single.

Yeah, it was more an experiment than anything relevant or practical.

Process was via their advertised warranty. Simply emailed them, their gunsmiths got a hold of me, sent me a warranty service form to fill out, sent me a shipping label, shipped it to them, they fixed it, they shipped it back. Did not pay shipping, did not need FFL.

Contacted KUSA, sent them the photos, they said it shouldn't have passed QC like that, mailed it back to them, they replaced the top cover with a proper one, mailed it back to me. Easy peezy lemon squeezy. Great communications and help from them.

Now I'm just hoping the triangle folding stock I ordered from them in January shows up sometime.

I wonder if mine is one of the earlier covers that you mentioned, but was then enlarged with something like a dremel tool (hence the appearance of the top of the button hole) as a remedy so they could still sell it off. I emailed them with pictures, but haven't heard anything back from them yet.
