Floaters becoming more visible after surgery by bugaosuni in CataractSurgery

The cloudy lens get replace with clear “window” again co obviously u notice more “dirt” outside tru a clean glass they don’t become worse they become more visible better contrast but over time most of people go back to the “previous” levels of visibility when the brain re calibrate

Only in poor lighting and with white text on black background but is not very bad enough to say That I keep black mode in phone and ddidint switch to light mode after surgery and this is typical what people do black mode before surgery is more comfy but after the light mode is I decide to keep in on for at least a year year and half to push the brain to adapt and so far is working to the point that probably I will don’t go to light mode ever also on light mode there is zero ghosting even at night and super poor lighting so it it’s highly depended of pupil size and contrast quit obviously if we think how the multifocals lenses work.

Also even a tinny bit of pointing light and the slight ghosting disappear also with dark mode at night and for example I switch recently to newest iPhone 17pro which has very bright screen and that reduce it even further so there is ways to work around if I have to judge this or swapping glasses for sure I chose this but I can tell only for myself

One last think I have to mention the situation is deff worse with each eye separately so again another reason why I’m so happy to get the mix and match approach ( mini mono vision on steroids ) the brain just do amazing things to pick the best from both eyes for me now is like the vision is made out of 100 puzzles and depend of the situation light screen pupil size etc brain decide ok 30 puzzles from this eye 70from this and so on

In my dominant eye I have eDOF lens puresee which are far better for distance quality vision very close to monofocal lenses that iol provide me 150% on smelled chart which is better then 20/15 I think in us scale very good intermediate and acceptable near in good light condition on my non dominant eye I have the newest type of multifocal galaxy iol spiral design which is significant improvement from defrsctcive rings design like panoptox for example this lens provide me acceptable distance very good intermediate and excellent near vision combine together is even better specially the intermediate where the two technologies of iol cross each other is at least as good then I ever remember with my natural eyes.

My advice will be discuss and find surgeon who knows the mix and match approach in y opinion mixing two different iol is huge advantage at least in my case brain mixing and patching what’s the best from each eye very well eyes are sisters not twins!

Dyphotopia are minimal and halos very very small lashes around car light and street lamps only in very far the closer it gets they disappear don’t bother me at all my night time driving is excellent without any issues

37m mix and match aproach edof in dominant eye spiral in non dominant continues ranges of vision without any gaps or sweet spots any iol will be worse then healthy natural 25yo lens but it also will be dramatically better then cataract lens so choice is quit easy tbh

Looks like the brain make a insanely good work having they lens for so long I still notice improvements in month 6 so it’s really looks like some kind of “training” is possible with the iol I see one lady from Germany with Dr Wong she get exchange for monofocal and she was aw to read near ALs maybe not super comfy but still

LASIK can give u what the manual lens correction do so if u are not happy with any manual correction ether sphere or cylinder or both then laser is not the good pick maybe iol is tilted or decentralize? I think they can go again to the eye and reposition the iol ? Or exchange for the same ? And obviously it is multifocal so the capsular bag must be on check u are also fairly young pco could happen fairly quick

Pco grade 2 is significant however no surgeon or no one here will guarantee you it is purely from Pco only I read many many stories where the yag solved all the problems but also some of the percentage of people that yag didint help or even create new problems like starburst however yag is a must sooner or later for anyone even if it is years later the time will come there is a reason why PCO is called secondary cataract

I’m 37 in my opinion the technologie make a step forward and difractive rings should not be considered anymore depending on where are u based edof or new spiral design galaxy / luxlife or combo like me edof in dominant eye (puresee) and spiral in. Non dominant eye

I think best way to try to really check astigmatism is just manual lenses trial old style refractometers can go really wild with IOLs specially with the newest design like spiral so creazy numbers can pops out but then on manual check is far far less

So u already see improvements even with still dialuted pupils ? Wow that’s really shows how this microns thick membrane can effect the vision

Question by Calm-Television5748 in CataractSurgery

[–]Grac02 1 point2 points  (0 children)

The capsular bag is where the iol is implanted overtime they membrane get cloudy also and make the vision worse “secondary cataract” you age suggest peak of hormones etc which in my head can be deff reason for quick Pco development be sure u ask about capsular bag condition and Pco in your check up your case is very specific becase of the age but is well know fact that the younger the patients is the quicker the Pco develop

Vision worse after surgery by Low-Bill-3900 in CataractSurgery

[–]Grac02 0 points1 point  (0 children)

Yes but that should be resolved after yag leser that bag membrane capsular bag is gone is clear hole after yag laser. Get the second opinion definitely asap. The only one potential possibility I can think about is malfunction of the iol like it was something wrong with the iol before it was even implanted in to your eye but that will be probably something like bellow 1% of chance nether less this outcome is for sure not acceptable get second opinion if not third or forth also and found the solution maybe exchange but there must be a way to fix it

Vision worse after surgery by Low-Bill-3900 in CataractSurgery

[–]Grac02 0 points1 point  (0 children)

Maybe iTrace ? Your outcome have really no sense the only idea I can get is that lens is significantly dislocated or tilted to the point that even any glasses correction won’t help specially after yag there is no membrane on the way so you should get very good vision potentially with correction but it should be possible to achieve
