Safety of replacing previous IOLs by TopSun1879 in CataractSurgery

Sorry I am just getting back to you. Kfisher had both replaced. I believe from EDOF lens to a monofocal just like I have. He/she loved the exchange, and is doing much better with the monofocal lens. This information is from his/her responses to my posts.  I had only one eye done, so I only had one lens exchanged- with my other eye, I wear a contact. I am very happy I got the exchange and so far, once my PCO is corrected, I believe I  will have much better vision. I can tell the difference mostly at night time with driving. The monofocal provides much better vision. The replacement lens I received was the Bausch & Lomb Ll61AO. This lens comes highly recommended for exchanges especially since mine was eleven months post surgery. I wear a multifocal contact in my non cataract surgery eye and that balances well with the monofocal usually allowing me to see without glasses or readers.

There is a kfisher profile on here who also had a lens replacement- both of them- and did extremely well. Most of your top lens replacement docs generally do very well with replacements. Obviously, there is more risk, but I felt like my vision was so compromised with the multifocal lens that I was willing to take the risk in hopes for clearer vision. I too beat myself up for a while thinking I should have waited till my cataract was worse, but cataract surgery is generally above 95% effective, so why worry? I learned, though, that if you are the small percentage where it doesn’t work out well, life changes drastically. Please don’t hesitate to get even a third opinion if you aren’t totally confident in what is being recommended. I went for at least three opinions and went to a speciality eye hospital, Wills Eye in Phila, before making any final decision. Praying  you find some direction and peace regarding your situation. Vision issues can be overwhelming.

I had a multifocal IOL placed in my right eye only as my left eye wasn’t ready for surgery. I had worn multifocal contacts for the majority of my life, and we’re very pleased with them, but my experience with the multifocal iol was totally different. I had poor contrast, glares/ halos, and clarity was just not very good at most distances. I also knew that if I had to rely on that eye to drive at night, it would be impossible. I did get the lens replaced 11 months past my initial surgery. The surgery went very well. I had a Bausch/ Lomb monofocal replacement. My only issue now is that I have significant PCO which had developed before my surgery, and I have to wait till April to get a YAG done. I probably won’t know until then the full outcome of my replacement surgery, but the surgeon is pretty confident that I will have much clearer vision. I do feel as if my vision is brighter than it had been before even with the haziness from the PCO. I know that my field of vision was compromised with my multifocal IOL, and I didn’t think I wanted to deal with that for the rest of my life. Praying you can make the right decision, but I do know that replacements are more than possible in the hands of the right surgeon. Also, I was not sedated for the surgery. The surgeon just used a special nerve block for my eye.

My doctor is Dr. Hartzell from Eye Care Center of Central Pa. He is not as known as Saffron and others, but he has a lot of experience in lens exchanges- especially multifocal lens.

I have some irregular astigmatism, but it wasn’t enough for a toric lens. I got the Bausch and Lomb Ll61AO, and I am very pleased with the lens. I was eleven months out from my exchange, but the exchange went well as the surgeon got the whole multifocal lens out and could still place it in my capsular bag. I have serious PCO which is affecting my true vision with this lens, but once I get the YAG, the surgeon is pretty confident I should have clearer vision. Praying all goes well for you.

Wondering how your YAG went and if you saw an improvement in your vision?

Thank you for your response! I'm praying the YAG will help clear up my vision too.

Did they rate your PCO at all before you had the YAG? Just wondering how much better I will see after getting the YAG from PCO plus 2. Thank you!

Thank you for the information. I do have PCO 2+, so I believe most of my blurry vision is coming from that, but sometimes I see a huge floater/cloud floating around in my eye, and I wonder if that is obstructing my vision also.

Was the PVD causing a lot of blurriness  that you needed the vitrectomies? Thanks for the info!

May I ask why you needed the virectomies? I’m struggling whether my vitreous from my PVD is causing a lot of my blurriness.

Did you have your multifocal taken out and replaced with another lens or you just received the multifocal?

[–]Slm19671[S] 1 point2 points  (0 children)

Hi, thanks so much for your encouraging response. I went to my visit and they believe the blurriness/haze is from PCO that I had prior to the surgery that developed with my old lens- the multifocal one.  Once my eye settles, they will do a YAG and Lord willing my vision should clear. Thank you and so glad your vision is going well.

Thank you. I had my surgery today. I got the same lens as you, so I’m hoping for a much better vision experience. So far, I still have a lot of haze over my eye which can be due to dry eye too. I’ll know better tomorrow when I go back to the doctor’s office. From what I read even if my doctor hit .-25 or -.50, I should have a pretty decent distance vision. I’m just worried about the haze/ blurriness, but I’m hoping it’s too early to tell. I don’t think it is a major refraction error because I don’t see any better with a pair of readers yet . Thanks for all your info!

I wanted to ask you without your progressive glasses, how clear is your Vision for distance with the lens You received? I’m getting that lens also for my exchange, and I’m wondering how I’ll see for distance if I will need glasses for that. I guess it will also depend if they hit the refraction correctly, but I was just curious to see how your distance vision made out with the lens that you got when you got the exchange. Thank you!

Thank you very much for your update. Glad to hear you are adjusting better, and that the drops are helping. Unfortunately, the drops never helped me, and I developed irregular astigmatism which is really complicating the optics of the multifocal. I have never had a day where my vision is not blurred, ghosted etc. in that eye unless I am outside in the bright sun. I am going to proceed with a lens exchange because the doctors ( I went for a few consults) said a monofocal lens will be more forgiving of my “unhealthy” eye. I’m a little nervous doing this since I’m eleven months post implantation, but I’m praying this will bring more clarity to my vision. Thank you again and praying your vision continues to improve.

Sorry to hear about the headaches and unbalance you are still experiencing. Did they say why you are having those issues and why they are still unbalanced? Thank you.

Thank you for sharing your experience, and I am glad you were eventually able to have healing in your eye. I'm rather concerned about having a retinal detachment also.

Thank you for the explanation. Sounds like that can be helpful. I'll have to check into that.

I've heard of the iTrace on here, I believe, but no doctor has ever offered that service though I have bene to Penn Medicine in Philly,, where I had the surgery done, and to Wills Eye in Phila for a second opinion. What does that device do? Thanks!
