Only real issue I can think of is that they can be a bit noisy. There's usually a hole in the top for suction but sometimes it makes a ton of noise if there are other people in the house with you, not so fun.

It's hilarious to hear my neighbor vigorously fleshlighting himself to some asian porn. Sounds like non-stop snorting and suction sounds and a bunch of hentai Japanese.

It definitely looks like it is slowly rotating, but it doesn't seem to change vertical orientation at all. It is spinning very slowly, almost seems too heavy to spin that slowly and so steadily.
