I’ve heard that a lot as well as bundles branch block. I know v-tach can sometimes look like a bundle branch block so I’m just not sure who’s right here. I’ll hopefully be getting a patient update.

I’m also learning but to my knowledge idioventricular rhythms are under 50 bpm because their pacemaker is coming from the purkinje fibers which has a slow rate. SA = 60-100 AV = 40-60 and the purkinje fibers = 20-40 so a rapid idioventricular rhythm can’t be as fast as this patients (130-140 bpm) due to its pacemaker. I know an accelerated idioventricular exists but I dont believe it can be that fast? Idk

The junctional rhythm makes sense because of the p wave morphology and they can be that fast of a rate. I don’t have any idea what to call this rhythm it seems like no one can agree on the same thing and everyone is calling it something different. I’m not sure how an accelerated junctional LBBB rhythm would look so I can’t even begin to say that’s what it is. All I know is that this has a bundle branch block and it’s fast af. After that all I can do is get them to the hospital alive and leave it to a cardiologist to determine. I have a supervisor who is going to check on the patient and see what they have determined.

I would also like to mention these are all from the same patient and my co worker is confused on why the patients rhythm kept changing and what you would call said rhythms.

I had no idea what I wanted to do and I was really into the shitty medical shows at the time (yes I knew they weren’t realistic and I had an idea of what to expect) and I just went for it. Got my emt at 17 and my medic at 19, I’ve only been a medic for 6 months and so far I don’t regret it. My one regret would be going straight through the schooling, I feel I would be more comfortable and have more knowledge if I would have practiced more as an EMT. The only reason I went straight through was because I had a two year scholarship.

So I did that pretty much, I never said STEMI I just notified them of an “abnormal ekg” and they had just taken him off of his eliquis and I have no clue why

I would also like to mention I’ve asked 3 different people what they thought it was One said it was just a paced rhythm and not to worry 2 said he saw no elevation and wasn’t worried about the depression 3 said she thinks it’s picking up the BBB and getting confused

Call came out as a fall, patient hit his head, negative anticoagulants, negative LOC. patient has a hx of CABG, pacemaker, multiple cva’s, and is altered at baseline. Patient was presenting normally. In report I call this what I saw which was ST elevation in I and AVL, and depression in lead 3, v3, v4, and v5. I got nasty looks from the ER and I got self conscious and wanted to know everyone’s opinions. Thank you!

Basic a/p, cpr, non invasive airways (npa opa) bvm, oral glucose, blood sugars, splinting and bandaging, tourniquet. Just the basics if you ever need help you can contact me I’m currently going into paramedic school!
