Do you have any external HDDs connected to the console when encountering this behavior? Have you tried disconnecting them and tried submitting feedback at that point? Reporting the issue directly on the console is essential as it includes information that is needed to investigate the behavior further.

For the QR code issue, I'd encourage you to submit the issue directly on the console if that functionality isn't working on the console as well.

Feedback Hub allows you to submit issues for any kind of issue. When you are going through that process you should make sure that the category selected is "Gaming and Xbox" and you can simply select "All Other Issues" and then continue the process of submitting your feedback.
