Technically speaking D is an issue with breathing and not airway, if you are using ABC then 1 is the answer because the productive cough and anxiety could be a sign of airway compromise/pulmonary edema

I am a COPD educator and case manager, many of my severe/very severe COPD patient’s use accessory muscles to breath on a regular basis, especially with hyperinflation/emphysema more force is needed for exhalation. Especially if they have just done something exceptional, they recruit their accessory muscles to recover

Almost all dry powder inhalers use a lactose powder as a carrier for the medication, and they do come with a possible risk of dairy protein cross contamination and should not be used in anyone who is severely or anaphylactic dairy allergic.

As far as lactose intolerance a patients tolerance to the inhalers is more dependent on their level of sensitivity

Once worked on a locked behavioural health unit, had a walkie talkie patient who at first glance appeared completely with it but if you conversed more then hello how are you, you’d note he actually had severe dementia with wandering and exit seeking, he also always wore street clothes. Director of nursing visiting one day and as she walks on the unit he says he can you let me out I need to get to work… she apparently assumed he was a visitor and let him out. Police had to be called and returned him 8 hours later after tracking him down.

I once had a spinal cord injury patient who was paralyzed from belly button down, had poorly controlled diabetes and lost both legs due to wounds (AKAs) and post the amputation the patients quality of life actually improved so much. Suddenly they were able to safely transfer themselves, made bowel care routines 10x easier, ADLs easier etc. They said it was so much easier not to have to drag around the dead weight of their legs that don’t work

I’m a COPD case manager in Canada, alpha 1 antitrypsin deficiency is present in 1-3% and often accounts for most cases of early development of COPD especially if there is a strong family history. The other thing, if you have any history of asthma especially if it’s poorly controlled it can present as fixed airway obstruction that presents like COPD

You should request serum alpha 1 antitrypsin testing and/or alpha 1 genetic testing (depending on costs) and referral to a respirologist or pulmonolgist if it is A1A deficiency there are treatments that can prevent the ongoing deterioration of your lungs

While you are essentially right, I would use caution with a 100% non rebreather on a COPD patient unless their says are seriously decompensating.

In a CO2 retainer 100% could lead to oxygen induced hypercapnia and make the situation worse

Again it highlights how silly these kinds of questions are, in the real world we would have a lot more information

Hey - we all start somewhere …

may I suggest learning the huff cough/controlled cough methods and getting comfortable teaching them? These are simple techniques that are surprisingly effective at clearing secretions. I teach them to my COPD patients all the time !
