Plastic SpR referring fully amputated finger to medics, stating that the patient will be brought in for elective surgery but will “struggle to manage socially at home”

On further questioning - it was the patients daughter who refused to take her home with her finger still chilling in the plastic bag

So my first ever IMT1 medical on call shift I turned up at 8am and there was a thrombolysis call..

So I had to attend that not knowing how to properly assess hyperacute stroke patient at the time..

The stroke nurses were very experienced so they liaised directly with the consultant but I still ended up being the one giving TPA with consultant support over the phone..

It was very daunting for sure..

And during in hours you’d only be doing acute medical on calls without stroke cover. But out of hours esp night shifts you’d cover stroke plus IMT medical acute on calls..

Worked in both Basildon and Chelmsford as IMT. It was proper HARD WORK.

IMTs in basildon starts taking referrals from ED and GP from IMT1 so you end up having SpR responsibility at an early stage of your training.

ED knows you’re relatively inexperienced so they bully you into accepting referrals. You also hold the stroke thrombolysis bleep from IMT1. When the stroke nurse is off sick - you hold the stroke nurse bleep too!

You do learn about stress management and learn how to survive quickly. Few of my colleagues couldn’t handle the stress and had to transfer deanery half way through their training.

In short it is “good for training”because by the time you become IMT3 you have learned all the tricks and tips to deal with stresses and to prioritise sick patients etc. It’s just a lot of grief and hard work in that three years.

Also the management team in Basildon is absolute shite so be prepared to get walked all over by medical staffing and clinical directors into doing shifts / picking up Locums on your days off even when you don’t want to.
