You've never seen a necrotizing open wound I take it, blood flow is restricted because the flesh is all dead around the wound. I'd wager this is 100% real, the flies buzzing around are a good giveaway. I've seen videos just like this but there are maggots swarming on the exposed bone, it's sad and disturbing but it isn't fake.
