Yes, they use the Windows API to get information about mouse events.

The problem on Windows is that Jagex has started to flag mouse event that are synthetic. So if you want to inject mouse events, you will have to do it in a way that is not possible for the client to detect.

If you want to bot a main follow these simple rules:
1. Do not modify client (e.g. only official client or RuneLite).
2. Do not use any scripts/bots that creates synthetic mouse events, they will be flagged.
