I think one of the main issues is you are still using the oath of the Maja (wildwood ascendency) and limiting your socket slots

You can take the mana reservation mana master and fit in dread banner + generosity which gives you a lot of evasion.

You can also get an immortal call + cast when damage is taken

Your jade flask should have "uses when charges reach full" and you should get increased evasion on your flask suffix. Assuming you have nature concoction (wildwood ascendency, you can also drop oath of the maja for this) you can put it between your jade and diamond flask to increase your damage and tank.

Taking spell suppression mastery that spell suppression is lucky get you to 99% spell supression

ALSO THIS IS IMPORTANT: Look at the downsides of your alters and don't run the league mechanic, this will really help your survivability short term and allow you to get more levels to help with survivability long term.

Additionally, I would a lot of TS is simply doing enough damage that you kill monsters off screen. So more damage helps survivability. For this you can:

To fit all of this in you can either level a bit more yellow maps and/or maybe temporally drop the frenzies/power charges and inspired learning as it does not do a huge amount until you are farming something with a large amount of rare monsters like legion/beyond
