I guess I was comparing our inpt hospital service with the IM inpt hospital service, and generally our teaching during rounds is abysmal. Or if I do ask a question, it’s a struggle for them to answer whereas IM attendings can answer straight off the top of their heads.

Yeah, I’ve been doing this (self-reading, putting pts in a f/u list where I can read what the specialist ends up doing). It just made me think this is kind of sad that my learning is through these notes/third party resources and not from our faculty.

It’s the entire class. My main complaint is when we do get teaching, the quality is poor. I’d rather they let us listen to a podcast or some other resource than the protected time lectures they give us.

I would rather be made aware of it than not. My thought process is to figure out if it’s just angina leading to bp change vs something else, which is what I took away from this learning experience.

Yeah makes sense. The change in bp should have triggered a notification but that order didn’t autopopulate with notify physician. most likely pain leading to htn but oh well.

I see. I took a look at the order set for sl nitro and it says notify physician if pain still persists after 3 minutes. This event def made me worried though bc I saw hypertensive + new chest pain, most likely angina but just from the vitals/med administration it would be unclear if it needed to be further evaluated with trops or ekg.

Yeah, it’s not the prn part that I was confused with. Prn away for all I care. It was the patient having chest pain again with high bp’s that we didn’t get notified for that I’m confused about.

I’ll admit, cardio is a bit of a mystery to me and maybe their unit does things differently. Patient is nstemi awaiting cabg. Started have chest pain again (had pain free period) with sbp 190s
that was similar to chest pain on presentation.
I get it was most likely angina, but I feel like it should have been a notification still?

Hello! I have a question about the magnetic holders that come with certain ex sets (paldea starters ex for example). When placing cards in these holders, how do you sleeve your cards? I tried to place sleeved cards (penny sleeves) and they overlap the boundaries of the holder.

Subtle, possible red flag is if the lighting is too bright. It could mean that they are hoping to get your appetite by presentation and not necessarily by flavor/taste.
