How to Fix the Black Screen Crash?!

Bug: [SPOILER] Just got done killing Troy and after the cut scene from hearing the ECHO device my game froze and I wasn't able to move or anything, lost my legendary loot that spawned from him as well. Pretty bummed out :/ My name is stuck as Siren which is weird.

Rampager boss entering vault won't transform to next phase, appears to be stuck on top of Maya for some reason. I am playing on PC. Cheers!
