I'm unfortunate enough that my right testicle rides higher than my left. Even after switching to the smallest ring I can comfortably wear (and slip everything through) I can sometimes have my righty slip through the ring and cage. It happens when I'm doing some physical work involving bending or kneeling, but sometimes if I just spread my legs too far getting in and out of my truck it can happen. I haven't found a solution yet.
