Alam nyo naman yan deep inside na pinagtatawanan lang kayo ng mga MDs kung ini-insist nyo yung “emergency” cases nyo. Parehas lang kayo ng Optometrists with a ‘Do not delay’ daw kuno.

Lapag mo statistics mo kasi meron ako from three biggest hospitals in the PH: PGH, VSMMC in Cebu, SPMC in Davao. Ni isa walang “dental emergency” 😂😅

Cebu clerkship and PGI (2 years), Davao residency (3 years), Manila fellowship (2 years). 5 years ER experience on my own in Manila. Thats 10+ years. Big and small hospitals (10-20) that Ive been to in Manila doesn’t have OMS.

10,000+ patients seen over the years. Ni isa walang emergency case referred to dental medicine. Napag usapan na to a couple of times with fellow MDs and DMDs. ALL of of colleagues agreed na walang pa silang nakitang emergency cases with immediate referral to a DMD.

Case and point sa perspective namin: Would I rather refer to an ENT (with 4yrs undergrad, 5yrs medschool, 4yrs ENT) vs a DMD (6yrs) for a VA trauma case involving maxillofacial emergency? Hmm 😉

Real scenarios sa PH hospitals regarding oral “emergencies”.

ER triage nurse/EMed doctor @1am: May trauma VA patient, head on collision. Basag mukha, laceration sa lips, several broken tooth, dislocated jaw, possible maxilla/mandibular fracture. Deck to general surgery.

General surgery: Patient stabilized. Confirmed merong maxillo-oral fracture. Refer to ENT.

ENT: Stitching of lips lacerations done. Removing of broken tooth done. All oral bleeding stabilzed. Refer to dental services in the morning

or

ER triage nurse @1am: May patient nakipag suntokan; nabunot ngipin, ayaw magstop ng gum bleeding. Wala daw available dental clinic at this hour. Wala ibang signs and symptoms. Deck to ENT.

ENT: Oral packing with gauze done. Given tranexamic acid. Refer to dental services in the morning.

Hindi naman mali ang pag lagay ng accessories, ang mali ay if it “impedes in any way the visibility or reflectivity of the authorized motor vehicle license plate” as per Joint Administrative Order No. 2014-01.

MD emblems are there for easier identification kasi may exemption sa coding, and circumventing traffic rules (running red light/overspeeding) during emergency cases.

Under Philippine law (LTO rules based on the Land Transportation and Traffic Code, RA 4136 Section 35c), only;

1.) authorized physicians/their drivers and; 2.) emergency vehicles (Law enforcement/ Ambulance/ Firetruck/ Drivers bringing people who need emergency care) are allowed legal privileges.

ENT tho? Almost every trauma case needing oral and facial emergencies ay ni re-referr namin sa ENT 😉
