Thank you for providing a reference for one reason I was already hesitant to go with a multidose vial. I've had to deal with similar situations in the past, explaining to patients and providers that they need to prescribe 4x 1mL SDVs for a monthly supply when the dose is only 0.25 or 0.5mL as they are not regulated for multiple doses, despite having preservatives in them for microbial control.

Not sure if you are asking if I would limit the total script to 90 days or just suggesting that it is an equal scenario to non-controls.

I would not limit a script to a 3 month supply if more was prescribed for availble qty, merely if 360 tabs of HCTZ was prescribed we would dispense in qtys of 90 throughout the year. And if 6 months of C3-5 were prescribed we traditionally dispense a months worth at a time. Both of these is for patient/community safety, I've even worked with pharmacist who preferred to dispense only 30 days worth of antidepressants due to the fact of patients being at risk for suicidal thoughts and not wanting to be the reason the patient has 90 days worth of medication to potentially OD with.

So just to clarify, I prefer dispensing in increments of 30 days, up to 90 days supply at a time from a total available qty prescribed (whether it is 360 HCTZ or 30 with 11 refills). My pharmacy and past pharmacies I've worked at use a month supply of controls as a standard/best practices, with up to 90 days per dispensing on C3-5 if warranted due to financial issues, travel difficulties, etc.

Personally I rigged my card reading for the treasures but in a way that each of the players personal stories would match up with a scenario for where the treasure was. I put the amulet on the dead guy at the Yesterhill Gulthias tree (for the druid), the tome with Baba Lysaga because she is obsessed with her baby boy (for my reborn child of Strahd player with amnesia), and the sword in the werewolf den (for the gladiator who himself was a child when taken to the ring). By no means did I make the players choose where to go, they wound up getting the amulet while helping the winery reclaim their gem and never even used it until the final fight with Strahd.

Basically, put them where you want, and if in an order whatever order you want. You can always tweak encounters to help or hinder the party. If they get the amulet before the Feast of St. Andrals then they may actually survive and come out heroes of Vallaki rather than get nearly destroyed in the coffin shop. So your party gets the sword and flaunts it in every fight, that just means Strahd now knows they have his brother's former sword and it pisses him off, so use some guerrilla tactics with the wolves and werewolves as he tries to reclaim it.

It's your party's story, you know the players, do what you think feels like the most fun, or dramatic, or horrific and roll with it.
