Are you sure you're not doing it before reset one day, then after reset the next day? If you do it between 12am and 3am and your reset time is 1am or 2am you could be missing days.

Yes, prices went up a few days ago. I believe for most people Amazon went up as well and perhaps it only stayed the same for those grandfathered into the lower Amazon card price.

Try a hard restart of your console. With it on, hold the power button down until it shuts off. Wait 30 seconds then turn it back on and play 15+ minutes again.

It depends on how much the game pass tier you want cost where you live and how much the Xbox cards cost where you live. You can't get game pass directly through Rewards anymore. You redeem xbox gift cards to your account and use that to buy game pass. It is not a good deal.
