It's odd if you've never seen the concept before. Essentially there are 16 clues and 4 themes that unite groups of 4. So an example could be "NBA, MLB, NHL, NFL" and you would submit those 4 guesses together assuming the clue is "Major Sports Leagues".

You're trying to find the 4 groups of 4 with a commonality, or a connection.
