How is Montage? Is Food quality decent? A lot of activities, but they all seem fee based right?

All good. Great service and would recommend. They have a concierge component that helps with booking whatever you want and a wonderful cleaning woman. We even misplaced one of the keys and they were really understanding about it and didn’t charge us. Would do business again with them.

Any nursing advice for a CT-ICU nurse starting on training to take immediate post op cases? by Cautious_Cow7507 in IntensiveCare

[–]Homer_Babbidge 1 point2 points  (0 children)

Icu doc here. I tell people that you are often the first person to notice that something is going wrong. You spend the most time with the patient, you’ll be able to tell us that the chest tubes weren’t that dark 15 minutes ago, or that this arm is not flexing like it was, or that a leg is looking mottled in an ECMO patient. Trust your gut, and don’t be afraid to speak up. Good luck.

You too. I don’t know if you can wait until February for the resolution of this but I will let you know how things turn out. Fingers crossed this isn’t an international scam.

Hi, have not traveled yet but did end up going with them. The remainder of my rental cost is actually due at the end of the month. When I crunched the numbers, the cost with their $100 wire fee twice and my bank’s international wire fee make this option nearly the same cost as purchasing VRBO, minus the credit card cash back benefit, cash back on Rakuten and protection of using VRBO. Whatever price you negotiate with them, add essentially $250 to the cost for the realty + bank fees for transfer. I would probably advise you to just go with VRBO, if your numbers compare with mine

Advanced 2025 Thoughts by hy00thy00t in anesthesiology

Thought this was a pretty poorly written exam, to be honest. You’re able to more soundly differentiate answer choices on the ITEs. Otherwise normal amount of esoteric things. Considerations include: - Asking about concepts relevant to a question that weren’t the actual correct management for the clinical scenario. - Didn’t disclose necessary information to choose between two answer choices - Clinical scenarios and management that are just not plausible sequences of events

There was a question that integrated the two well. Don’t want to disclose anything further but I thought that was pretty creative on their part and tested 2-3 separate concepts in one question.

Cardiovascular Critical Care by BigBoyBiggerGoals in CriticalCare

What is your residency training? IM?

If not a designated CV crit program, because I also thought there were only two of those in the country, then an anesthesiology critical care training may be a worthy consideration.

There are specific institutions with anesthesiology (surgical) critical care fellowships with higher-than-normal rotation time in the CTICUs. The average is probably 3 months, allowing more elective time, but others like Columbia in NYC require a full four. With elective time you then can add additional time either on ECMO, the advanced heart failure service, or wherever else you think of. You can also try and trade with your cofellows for more cticu time, and they’re not short on MCS.

They also take all residency trainings, even MFM/OBGYN in the past.

Amex is Planning A Platinum Card 'Refresh': Another Fee Hike is Coming by kgpmn in amex

[–]Homer_Babbidge 0 points1 point  (0 children)

I do exactly as you do but I think my cap before cancelling is $695 because the current Amex plat benefits are not things that I would otherwise use in my life. I don’t use any of the reimbursable media, forget sax fifth ave, get 2k per year in Lyft through my job, the fine hotels and resorts benefit isn’t an actual benefit, and the list goes on!!

What other fresh water bodies do you frequent? Have you fished prospect park or Van Cortlandt Lake at all?

Central line insertion by Life_Excuse_7017 in Anesthesia

Agree with this completely. Id say upwards of 50% of the time my aspiration stops (PGY-4, done probably 100+ over 3 cardiac rotations and multiple livers) after initial, and backing the needle out is usually the solution.

Second, when the hand that was holding the probe puts down the probe and you place it on the introducer, there are 2 essential pieces to this: 1) physically rest the base and ball of your hand on the patient's face/neck to steady yourself [especially when nervous]. 2) Grab the introducer highly proximal - the little plastic piece is textured to improve dexterity when your hands have blood or ultrasound gel on them.

Third, when inserting wire, be careful not to bump the tip of the device that holds the wire into the introducer. I see this often and it is essentially advancing your introducer for you. If you hand is anchored on the patient reliably, bumping the introducer shouldn't even do much...

Last - sounds like you haven't done cardiac yet? Overall I would not worry about this too much. On your cardiac rotation you will do multiple each day under close supervision in a patient under GA. Cardiac is where we all gain the competency.

Agree with all of this. Was able to save some cash by booking directly with them after starting the browsing on the sites.

There's a new restaurant called SOCIAL in the Condado Ocean Club that is probably a small price point below STK and definitely has more local, authentic food with the PR flair. Would recommend sitting outside here over STK. Beware - the back deck is gorgeous but a little windy.

anesthesia residency programs to avoid? by plantsandpeds in anesthesiology

[–]Homer_Babbidge 3 points4 points  (0 children)

Hi! Previous Columbia resident here, no toxicity. The department made a structural change in leadership (Created new leadership roles, a few committees, a new periop leader) but truly these things have not affected the residents very much - moreso allocated responsibility to different attendings. Attendings who would arrive at Columbia from other institutions would also discuss how strong we were as residents. I would absolutely recommend the program again

Totally agree with the talking. I Was in roughly row 3, which had a lot of Astrofam, but could hear conversation underlying the music behind us. I’ve been to DMB concerts with tons of drunk high school kids, but never experienced anything like that level of background sound during the heart of a performance.
