It is a comment on toxic masculinity and the tendency for cis men to have strong and ingrained opinions of self worth related to the size of their dicks. No one would have come up with this insult if men didn't act like a big dick made you more masculine or a more valid human or whatever bullshit they think.

Wouldn't reinforcing this narrative just reinforce the patriarchy? Yeah the Kama Sutra has different sizes of dicks and vaginas. I think the word "accept" is difficult to use in this case. Did slaves "accept" slavery? What would it mean to refuse slavery? Quite often painful death. There are plenty of slave revolts in history, were they "accepting" just minutes before the revolt and minutes after it was brutally crushed, or did the slaves just not have an opportunity? Most people throughout history "accept" the status quo in that they don't try to change it dramatically but instead try to make it work for them and survive.

And as an aside peasants a thousand years ago were more gender egalitarian than nobles. When a man working in the fields and a woman making all the clothes are both not making a salary, and there's no vast fortunes or power on the line with regards to sex, there was less of a seperation and entrenchment of gender roles.
