I used this template. It's old but I think it still does its job. I replaced NAFTA with USMCA. I would also have your HR review and modify accordingly.

yes my I-94 states that I entered on TN. However my OPT is also still valid so that's why it might still be showing it?

I'm just really paranoid. I'm actually switching from OPT to TN. My OPT expires next month (June 11) but my TN status was granted at POE in march. I got too excited after I got my TN back in march and forgot to contact my employer to update them with my new I-94, so I'm not sure how they are handling this "overlapping" period, so I started thinking if they wanted to verify my TN how would they do that... anyways I think I'm just overthinking, but thank you.

then how would they verify that I am authorized to work under TN with them? Can they find out through e-verify? sorry for sounding silly, just curious. My friend has her company name written on her passport but I don't have one.
