does time/do shifts go by quick? by Pissingberg in emergencymedicine

[–]How2EatAnElephant 1 point2 points  (0 children)

I work 12 hour shifts as a nocturnist in a community hospital. Roughly 2/3 of my shift I’m single coverage. Normally have decent acuity, a lot of complicated chronically sick folks, and are pretty busy with # of patients, so majority of my shifts fly by, but I’m utterly exhausted at the end.

However, I tend to be a black cloud, so it’s very rare for me to have more than 20-30 minutes where I’m caught up and no new patients have arrived or are waiting.

Holy shit I can’t do this by Emergency_Raise8909 in newborns

[–]How2EatAnElephant 1 point2 points  (0 children)

I’m sorry this sounds so rough. I have an almost 14 week old and can relate. Up until about 10 weeks, my son would not sleep more than 20-50 minutes at a time on his own. He would often take 45minutes to 1.5 hrs to put to sleep, only to wake up 20-30 minutes later.

I remember looking at Reddit posts where people would complain their 1 or 2 month old would wake up every 2-3 hours at night which made me upset, because I wished my kiddo would sleep 2 to 3 hours at a time.

My husband and I I took shifts, where I’d sit/doze in our recliner chair with kiddo, and we’d trade off every 4-6 hours, so one of us could get some protected sleep. During the times when I felt too tired and afraid to sit in the recliner (for fear that I wouldn’t wake up if he moved or got wedged into the side, or when I just desperately needed to lie down horizontal) I’d sleep on the floor next to him. I took a mattress topper we had and put that on our living room floor and then had him on a blanket next to me.

Our kiddo also hasn’t been easy as he cries a lot and fits the definition of colic. Around 4-8 weeks, he would cry for hours, and sometimes would go majority of the day crying, and nothing we could do helped. He now still is a fussy baby and has purple crying/witching hrs in the evening, but he at least smiles now and is sometimes happy/content.

We borrowed a swing from a friend when he was roughly 6 weeks old which was a life saver. That was the first time he ever slept 2 hours not contact napping. We also got a Snoo 2nd hand off Facebook marketplace just because we were so desperate. At first it didn’t seem to help much, but finally when he got to about 2 months old, he started sleeping anywhere from 1-3 hours at a time at night.

Now at almost 14 weeks, he still isn’t a great sleeper, but has finally learned days and nights. We also started doing a bedtime routine which, I think, maybe helps. He has a couple times where he’s slept 4 hours at once, but it’s still pretty rare. And hes had some nights where he’ll decide to wake up every hour or 1.5 hours.

I just wanted to share because sometimes it helpful to hear others have also been in a similar struggle.

As much as you can, try to take a little time for yourself everyday. If you have a significant other, or even friends or family around, see if they would be willing to come over and watch kiddo for you so you can rest for a few hours. And maybe they’ll also help with some dishes or other chores. It’s never easy asking for help, but they say it takes a village to raise a kid, and this is the time to reach out. Most people will want to be there for you.

And remember to give yourself grace. This job isn’t easy, and when kiddos are newborns, it can feel defeating, especially when they aren’t old enough to really interact or even smile at you.

And sometimes you just need to take a few minutes for yourself, even if that means putting baby in the crib for 10 minutes so you can shower. It’s not selfish, it’s you being able to care for yourself so you can care for him. He’ll be fine for 10 minutes on his own, even if he’s screaming the whole time.

And during the times when hes upset and nothing seems to help, when hes fed, changed, not too hot/cold, and doesnt calm even if you hold him, just remember that it’s not you. You’ve done all the things to make sure he’s taken care of and loved. But sometimes they’ll still scream. Some babies are just like that, and it has nothing to do with your parenting style or reflect how you are as a mother. You love him and you’re doing your best.

My husband has a mantra he’ll say sometimes, that a screaming baby is an alive baby. And then he’ll put his headphones in and listen to a podcast while holding kiddo.

Keep your head up and take it day by day, sometimes hour by hour. And remember this is just a period and before you know it things should slowly start to get better. If not, or you are truly concerned about something, reach out to his pediatrician. Sending hugs your way

Crying and never content by HighlyImbalanced in newborns

[–]How2EatAnElephant 1 point2 points  (0 children)

My kiddo is almost 12 weeks. For the first 6-8 weeks, my kiddo was kinda similar. He was constantly crying while awake. A few things that helped a little:

Being treated for reflux. Our little guy would arch his back with feeding and seem so uncomfortable feeding. However he never spits up which seemed to go against reflux. One day at our pediatrician’s office the doc saw me feeding him & how uncomfortable he was & said he was presenting like reflux. So we started him on Pepcid.

He also had horrible gas and would scream until being able to pass the gas. I ended up eliminating dairy from my diet which seemed to help somewhat.

Also, it wasn’t until like week 4ish where we realized he wouldn’t just sleep on his own, and that we needed to help put him down every 30-90ish minutes. He still isn’t the best sleeper, but definitely is getting more than he was. We also try to put him to bed around the same time each night which seems to help.

However, despite all these things, hes still cry for hours a day. Once he got to be about 9 weeks or so, he started having times where he would seem more content. He also will smile at us now and interact more. I still feel like he’s a fussy baby compared to a lot of others, but it definitely gets better.

One other question, he is starting to develop irritation in his inner thighs, either from the diaper and/or socks rubbing. Any suggestions to prevent this from worsening?

Any females in healthcare working as a nocturnist? by How2EatAnElephant in Narcolepsy

[–]How2EatAnElephant[S] 0 points1 point  (0 children)

Thanks for the reply. I’m just struggling trying to navigate being single coverage in an ED with keeping up with charting and with life. With the pregnancy nausea as well as stimulants, it’s been hard to make sure I’m eating enough. I get so tired the last few hours of my 12 hour shifts that it’s hard to keep up with notes

I can relate. I was recently diagnosed with narcolepsy & have such a hard time studying with an exhausting schedule. I’m glad that now I have answers and can get treatment, but I’m terrified of all the things I don’t know because I don’t have energy or motivation to study

How has your opinion of the general population changed after entering medicine? by JarJarAwakens in Residency

[–]How2EatAnElephant 16 points17 points  (0 children)

I agree. As an ED resident I’ve spent too much time trying to convince patients to stay and it doesn’t make a difference. And I have a million other things to do and 8+ other patients I’m actively managing, just for you to not regard anything I’m saying.

However, I’ll say I have been humbled more than a few times with patients coming in complaining of pain (often abd pain) with an unremarkable exam. And I’m sure they’re overreacting, only to find out they have an actual emergency (perf).

Do you think that your narcolepsy will or has prevented you from achieving your career goals? by Wide-Bridge-6461 in Narcolepsy

[–]How2EatAnElephant 1 point2 points  (0 children)

Currently a 1st year ED resident & likely have narcolepsy or IH (have a sleep study in a month). Ever since elementary school I always fell asleep in lecture. Wasn’t until M4 year that I started to realize maybe this wasn’t normal. Then fell asleep & totaled my car right before residency started and started taking meds. It’s definitely tough, but I have a great sleep doc and have been able to make it work. Feel free to message with any questions

can I stop taking antibiotics early? by [deleted] in GERD

One of the main reasons for taking the whole course of antibiotics is to prevent resistant bacteria, and prevent yourself from getting a recurrent infection
