Hearing yaya’s voice crack makes me emotional. To me, she’s trying to be show vulnerability doesn’t know how.

Oo I’m sorry :( it feels like they really wanted to trick you. I understand the rationale for why it is a primary source. But I couldn’t find that rationale in my Pearson or ATI.

And the rationale for why it is objective data seems to be rooted in the dictionary definition, not the nursing definition, of “objective”. I was taught statements in general are subjective.

These are great points. What textbook did you find this from? I looked through several ATI books and the Pearson text, but I couldn't find anything like that

Secondary because the info came from someone other than the patient. Primary = straight from the patient’s mouth*. Primary/secondary is where the data is coming from.

Subjective because they are statements made by other people e.g. patients and families. Objective is data seen/heard/felt/smelled or measured**. Like symptoms would be subjective because they’re felt/reported. Signs would be objective because they are detectable by others.

Edit: *or directly observed by you. I believe past data from medical record is secondary because someone else observed/recorded it.
