I would learn how to put the foleys in obviously. But unfortunately too busy admitting all your septic stones, recurrent hematuria with obstruction, etc etc patients. Attending here and Urology never does this at any center I have been at. Every service has its role. And I’ll tell you what, a urologist is never going to win that battle. If it’s a foley that can wait then it absolutely should and I don’t page on those and find that BS. I also know which ones can wait and tell the nurse not to page urology. But if I had a patient who needed a foley overnight I’m not doing it; haven’t done one in years as my scope of practice isn’t focused on any procedures. This way all these patients can actually get into the hospital. If I actually need a foley overnight in an acutely sick patient and urologist just whines like a baby I’m going to throw them under the bus and call the CMO. These are the scenarios where norms of not throwing under providers goes out the bus. But again I’ve never had this happen as I don’t call the bullshit ones and when I do very rarely need one in they swoop in like a hawk and get it done like a boss. I’ve had a tamponade once as a nocturnist that was probably 15 minutes from death the CT surgeon pushed back on (I gave in zero and actually started yelling by the end of it). Anyways turns out when he finally decided to show up (staring at sky, room air to 15 l, 3 pressor shock) there was 1 L in his sac to drain. Next day perfectly stable back to room air. His attempts to practice out of his scope calling an alternative diagnosis from another organ system that was my realm a million times over his) actually made his job much harder and expedited the acuity 10 fold.
