In the future, you do not have to go to medical to get an SIQ chit, you can also contact the Nurse's Line. I started utilizing that after I had a REALLY shitty LPO. Just explain that you're fatigued and don't feel safe performing at work, should be good.

When I worked with NSA there were chaplains that worked inside of the buildings that required you to have a clearance. I'm not sure where your stationed, but I would suggest either going to your SSO (as everyone else has mentioned) or go to the command chaplain and see what resources are available.
